Insurance Plans and Payment Policy 

Georgia Sports Medicine participates in most health insurance plans and is a preferred provider on many plans. Once we have accurate and complete health insurance data is obtained, your insurance claim filed within 24 business hours following an office visit. It is not necessary to bring an insurance form with you at the time of an insurance form with you at the time of an office visit.

Complete insurance is required at the time of an office visit (including the name of the insurance company and policy number). Usually, all the information our office requires is found on your insurance card. If your office visit is due at an inquiry on the job, a proper authorization is required to allow GSM to directly bill your employer's insurance carrier.

For those of our patients that are covered under health insurance or other governmental programs, GSM is required by law to collect any co-payments, coinsurance or deductibles that are specific to the benefits that you receive. It is the responsibility of the patient receiving services to meet the financial terms outlined in their benefit plan. Because of the high costs associated with printing of statements and mailing, we ask that our patients know the terms of their insurance coverage and pay for any co-payments, coinsurance and/or deductibles at the time of service. For your convenience we accept Visa, MasterCard, cash or check.  

Patients without health insurance are required to provide payment in full for services received at the time of service. Uninsured patients are expected to pay the estimated cost upon check-in before the patient will be seen. Any remainder or over payment will be collected or refunded at the completion of the visit before leaving the office.

Fees
Fees charged are reviewed regularly. A specified period of post-operative care is sometimes included in the charge for surgical procedures. This includes routine post-operative treatment.  However, unexpected problems or unrelated conditions that might arise are not included. The “fracture care” charges include follow-up office visits sufficient for care of the specific fracture but do not include any x-rays, casts, splints, or other orthopaedic equipment.
